I. Requirements for an AFFILIATE MEMBERSHIP in the PCTA are as follows:
Accomplished PCTA (Philippine Cable Television Association, Inc.) Application Form for Membership duly signed by the applicant or its duly authorized representative (in duplicate form).
Articles of Incorporation and By-Laws together with the Certificates issued by the Securities and Exchange Commission (photocopies).
